I have started the tedious and extremely time-consuming task of spotting hot topics.
I will be adding to this list over the next week or so: make sure you keep returning.
But you can make a start now.
Read any Green-tops, NICE guidelines, MCQ answers, TOG and OGRM articles relating to the topic.
Make out an essay plan.
Then make out cards!
I am producing this by amalgamating the lists from:
GreenTop & NICE guidelines, past essay questions, TOG articles and OGRM articles and BJOG editorials.
I have tried to prioritise, so that the ones I most fancy come at the top.
I have put in neonatal screening and advise that you read it.
It has not appeared in the essays, but has changed radically in recent years, so is a likely topic.
Other neonatal topics like jaundice, resuscitation and examination could feature.
Have a session to cover them with a friendly paediatric registrar.
Fragile X syndrome has not featured in the essays, but must do so one day.
Vitamin supplementation and pregnancy was the subject of the RCOG's SAC Opinion Paper 16 published in 2009.
Folic acid fortification of flour was the subject of the RCOG's SAC Opinion Paper 4 published in 2003.
I have written a model essay.
Cancer is a likely essay and one of those things you must prepare, but revise in the days before the exam.

Cancer of the cervix has appeared in various forms in 1999, 2002, 2007 and 2008.
I think another topic is more likely:
ovary, endometrium, vulva, screening and palliative care.
There are various articles in TOG and OGRM, including a "model essay" about a woman requesting screening for ovarian cancer.

The RCOG published SAC No 12, "Targeted Therapies for the Management of Ovarian Cancer" in December 2008.
You can't pick a single hot favourite from this lot.
You need to prepare for screening and the management of the common cancers, particularly ovary.
Watch out for that difficult one about prognostic factors in choriocarcinoma.
It came in 1998 and could come again.
It is dealt with in MCQ 6, question 19.
Make out a card with the FIGO scoring system.
Put it into your bundle of things for last-minute revision.
Palliative care came in 1999, 2003 and 2007, so it will return soon.
Definitely one to prepare - there was an article in OGRM is June 2009.
It is a favourite as you need to know about palliative radiotherapy and chemotherapy, pain relief and relief of nausea.
Also about support systems: GPs and community nurses, Macmillan nurses, and hospices.
You also need to think about legal stuff; the right of the individual to decline treatment, making a will etc.
Organ donation is not usually an issue, but worth mentioning as negative statements often get a mark.
You need to involve religious groups for those with religious faith.
And you need to think about care of the relatives.
Contraception is a regular topic.

There has been a contraception essay in 9 of the past 12 years.
So, it features on average once in every 1.3 years.
This makes it likely this year.
I think you need to prepare all of these, with the exception of LARC.
Emergency contraception has changed dramatically with the arrival of Ulipristal.
If the examination committee had heard of it when setting up the questions, it would make EC likely as no other topic has changed much.

Topics derived from the "Green Top" Guidleines.
If a topic is important enough to merit a "Green Top", it is almost certain to be the basis of an essay at some time.
For example, "Early pregnancy loss" featured in the exam in 2002 and 2004.
But there was an updated guideline in 2006 and a TOG article in 2007.
The subject duly reappeared in the essays in March 2008.
If you look at "Alcohol & pregnancy", there is a guideline from 2006.
There is advice from the Department of Health and a load of media chatter.
It has never appeared in the essays, so must do so in the near future.
"Venous thrombo-embolism" has not appeared in the essays since 1999.
But we have a 2007 guideline and TOG articles in 2006 and 2007.
It has got to come soon!
I also fancy infection in pregnancy, which could be anything from chickenpox and parvovirus to malaria and tuberculosis.
Blood transfusion has a new guideline for 2008.
It appeared in the essays in 2002, so is likely to do so again in the near future.

Things to read up the night before.
Primary amenorrhoea is one of those things you should always read up the night before.
It last appeared in the essays in 2001, so it is a probable until it has reappeared.
Similarly you won't remember the facts about intersex for more than a day or two.
Hirsutism is worth reading again.
Cancer staging is a must.

Folic acid has never featured in an essay.
Fortification of flour is almost certain to be introduced in the UK in the next few years.
This will make folic acid too historic for an essay.
If I were on the exam committee, I'd get it in now before it is too late.
Adolescent pregnancy appeared in the 2007 British Congress.
There has also been a 2007 TOG article.
It last featured as an essay in 2004, so will resurface soon.
You need to know the key facts from the maternal mortality report.
But I think it will be in the MCQs rather than the essays as the report it only came out late last year.
